Insurance Information

We are pleased to provide this list of insurers with which The Valley Hospital contracts. Please note the following:

1) This list only reflects the insurers and managed care organizations that have contracts with The Valley Hospital. Your physician may or may not have contracts with these plans. You must contact your physician directly for this information. If The Valley Hospital accepts your plan, there is no guarantee that your physician accepts it. (The Valley Hospital and its physicians contract separately with insurers.)

2) As a courtesy to you, we provide links to each plan's web site. Valley Health System is in no way responsible for the accuracy of the information on these sites, nor are we responsible for any of the sites' functions. The sites are the property of the sponsoring companies, and inquiries about the sites should be directed to them.

3) When you click on a plan's link, a new window will open. To return to the Valley site, simply click the "X" in the upper right of the window, or click away from the insurer's window in an area where the Valley site can be seen.

Amerigroup 
The Valley Hospital terminated it's agreement with Amerigroup effective 9/23/05. Under New Jersey regulations, Valley Hospital will continue to abide by the terms of its Amerigroup contract until January 23, 2006. Under these same regulations, Amerigroup must notify its members and participating physicians of Valley's termination and provide instructions as to the impact, if any, Valley's termination will have on their access to care and participation status.

The Valley Hospital is pleased to announce that it has signed an agreement with Americhoice effective January 1, 2006.  Patients wishing to enroll with Americhoice can call NJ Family Care at 800-701-0710.  Please follow the prompts for applying for NJ Family Care and tell them that you wish to switch your current coverage to Americhoice.  They will assist you in making this transition.

GHI HMO
The GHI-CBP Plan (Comprehensive Benefits Plan) requires its members to receive care from NY providers only. The ID card will have CBP on it and will specify "The City of New York Health Benefits Program." Please note that even though Valley Hospital has a contract with GHI, if you are a member of the CBP Plan, your coverage will be limited or services will not be paid altogether if you receive care at Valley Hospital.

* Insurers that offer HMO (gatekeeper) products may place restrictions on where patients can receive certain outpatient services (i.e., lab services, radiology, physical/speech/occupational therapy). If you are seeking one of these services, please check with your insurance company before making an appointment at Valley. In addition, please note the information below.

Outpatient Radiology Services:
• Cigna —Cigna utilizes American Imaging Management's (AIM) radiology network for HMO, Point of Service and PPO products. Patients wishing to utilize their in-network benefits must use an AIM provider. Valley is not an AIM provider at this time. Cigna indemnity products have no restrictions on outpatient radiology services. Cigna HMO/POS/PPO patients may utilize Radiology Associates of Ridgewood's free-standing office in Waldwick.

Outpatient Physical Therapy/Speech Therapy/Occupational Therapy:
Patients with an HMO or POS product from Cigna must use an OrthoNet participating provider. Valley does not participate with OrthoNet, so patients should call Cigna to get a list of OrthoNet facilities.
